A brief synopsis of recent work
My focus as clinical nurse manager for the Public Health Nursing service in Dublin, Ireland has been to institute a change process related to screening of Older Adults in our area. Seventy-seven nurses were trained to use an assessment tool measuring frailty markers (falls/nutrition/dependency and cognition risks) in Older Adults. This assessment was provided to all clients seen by the nurse, regardless of referral route or nature of symptoms. The concept of equity served as the initial framework for developing this new process (Hanafin et al. 2004). The data derived from the collective assessments is currently being analysed to identify population health trends that may inform community service planning.
In addition, our work has highlighted the importance of continuity in care. The assessments allowed for opportunistic case finding of conditions requiring referral to specialty services, revealing many gaps in our service. Where equity and continuity of care had previously been viewed as diametrically opposed concepts, we concluded that these terms are never mutually exclusive.
